Completed ACORD 130 Application: Please complete all fields on the application.
No Loss Letter (Sample) or Loss Run Report: For start-up companies and those without prior coverage please provide a signed copy of the no loss letter. For those with prior coverage please provide a loss run report from the previous insurer.
Opt-In Letter (Sample) for Sole Proprietors: Please remember the statutory payroll for sole proprietors who opt in for coverage is $38,000.
DIA Form 153 (Corporate Officer Opt-Out): Please submit this form for all corporations with officer and principals who choose not to be included on the policy.
